The CMS-2744 Annual Facility Survey

At the start of each calendar year, Medicare-Certified dialysis facilities and transplant centers are required to complete the CMS-2744 Annual Facility Survey. This survey provides a final count of all patients and treatments provided by a dialysis facility or transplant center during the prior calendar year. It also includes important facility and patient demographic information, such as Medicare Coverage, Vocational Rehabilitation, and Facility Staff counts.

Facility representatives complete the CMS-2744 Form in the End-Stage Renal Disease (ESRD) Quality Reporting System (CROWNWeb). The system uses data from the Facility Details, Admit/Discharge, Treatment Summary, and Patient Attributes screens to pre-populate CMS-2744 fields. Users must ensure that all patient records were accurately reported in CROWNWeb to assist with eliminating CMS-2744 error messages that may display and must be corrected before the form is finalized.

Facility representatives initiate the CMS-2744 completion process in CROWNWeb, by generating a form and submitting it to their ESRD Network for acceptance. The CMS-2744 Form goes through various stages before it is complete. Users must monitor their forms’ status. The form is complete once it is in a “Finalized” status.

Resolving CMS-2744 Errors

When completing the CMS-2744 Form in EQRS/CROWNWeb, the system displays error messages to help identify invalid or missing data. These messages are removed after users correct the data, and “re-generate” their CMS-2744 Forms.

To assist with this effort, EQRS/CROWNWeb has five built-in CMS-2744 reports. These reports help to identify any patients who were added during a survey period, patients’ Medicare enrollment status, patients admitted to the facility at the end of the survey period, patients hemodialyzing more than four times per week, and patients’ vocational rehabilitation status. Guidance for Submission of CMS-2728/CMS-2746

This message is being provided on behalf of CMS and the Division of Kidney Health (DKH).

In November 2019, the Office of Management and Budget (OMB) approved revisions to CMS-2728 and CMS-2746. At this time, those revisions are not yet reflected in CROWNWeb; however, the most current forms will be made available with the planned implementation of the updated EQRS system. In the interim, CMS will continue to accept both versions of the forms. REMINDER: HARP Account Needed for EQRS Users

CMS rolled out the Healthcare Quality Information System (HCQIS) Access Roles and Profile (HARP) portal for EQRS users in February 2020. HARP is a secure identity management portal, provided by CMS, that replaces the Enterprise Identity Management (EIDM) system. All EQRS users must complete a HARP account setup. EQRS System Deadline Updates

CMS is extending the March 2, 2020 deadline for the following areas of data submission to the ESRD Quality Reporting System (EQRS): In-Center Hemodialysis Consumer Assessment of Healthcare Providers and Systems (ICH CAHPS) Attestation, Clinical Depression Screening and Follow-Up Plan, and December 2019 Clinical Data Submissions.

The March 2, 2020 submission deadlines for data supporting these areas has been extended to March 16, 2020 at 11:59 PM Pacific Time. Please note that this date adjustment is for the March 2020 data submission deadlines only and will have no effect on previous or future due dates.


Clinical Closures

Clinical closure dates apply to all Collection Types (Hemodialysis; Peritoneal Dialysis). Additionally, clinical data submissions apply to all submission methods.
