Pain Assessment and Follow Up Reporting

In order to comply with the requirements of the PY 2018 QIP, providers must submit Clinical Depression Screening and Follow-Up Plan information for each eligible patient at least once between 1/1/2016 and 1/31/2017. This information is:

The steps required to add Clinical Depression Screening information are:

  1. Click the Clinical tab. The Manage Patient Clinical Values screen displays.

  2. Click the Patient Reporting sub tab. The Clinical Depression Screening and Follow-Up Reporting screen displays.

  3. Click the Pain Assessment sub tab. The Pain Assessment and Follow Up Reporting screen displays.

  4. Enter the desired Facility CCN or Facility NPI, and then click Go. The Facility DBA Name displays.

  5. Click the drop down arrow in the Assessment Period field and select the desired date range. The selected date range displays.

  6. Click the drop down arrow in the Patient field and select the desired patient. The patient name displays.

  7. Select the desired Pain Assessment radio button, and then click Submit.

  8. CROWNWeb displays the Last Submitted Date and Time.
