Admit a New Patient

 

CROWNWeb allows users to admit and discharge new or existing ESRD patients from a facility, including the ability to admit and discharge a patient on the same day.

When a user is admitting a patient, the system performs a search for exact match or "near match" patients within the system. Matches are based on the following six identifiers entered on the Admit a Patient screen:

If there is no exact or near match in the system, a new patient is created.

The steps required to admit a new dialysis patient at a facility are:

  1. From the Home screen, click the Patients link. The Search for Patients screen displays.

  2. Click the Admit Patient link. The Admit Patient screen displays.

  3. Enter or select the required patient information, and then click Next. The Add Treatment Information screen displays.

  4. Enter or select the patient's treatment information, and then click Next. The Edit Patient Attributes screen displays.

  5. Enter the patient attributes data, and then click Submit.

Note:
The data entered on the Patient Attributes screen will carry-over to the CMS-2728 and CMS-2746 Death Notification forms. Complete all required information on the Patient Attributes screen prior to creating a CMS-2728 form.

  1. The View Patient Attributes screen displays the message "View Patient Details SUBMITTED."
