Print a CMS-2728 Form

All CROWNWeb users can print a CMS-2728 form for a patient in their scope. In order to print a patient’s CMS-2728 form, the form must exist in a “Saved” or “Submitted” status. Also, Adobe Acrobat Reader must be installed and configured on the user's computer to preview and print.

NOTE:
Users will not be able to print a Saved CMS-2728 form until all required information with the exception of the Patient and Physician signature dates are entered on the form. Users can save the form multiple times, but the print option will not be enabled until all required fields are completed.

A copy of the completed, signed CMS-2728 Form must be stored in the patient's medical records file in accordance with the medical archival rules in effect in your state.

To print a 2728 form:

  1. From the Home screen, click the Patients link. The Search for Patients screen displays.

  2. Enter the patient criteria, and then click Search. The Patient Search Results screen displays.

  3. Click the desired patient's CROWN UPI link. The View Patient Attributes screen displays.

  4. Click the 2728 link. The Manage 2728s screen displays.

  5. Click the desired Dialysis link in the Existing 2728 Forms section. The View Form 2728 screen displays.

  6. Scroll to the bottom of the screen, and click the Print button. The Cognos Viewer screen pops up displaying the 2728 Form.

  7. Click the printer icon. The Print screen displays.

  8. Select the desired printer, and click OK.
