Generate a Missing Forms Report

 

You can generate a Missing Forms report that contains a list of CMS-2728 and CMS-2746 forms that have not been completed based upon the patient treatment information.

To generate a Missing Forms report:

  1. From the CROWNWeb Home screen click Reports. The Facility Reports screen displays.

  2. Click the Missing Forms Report link. The Missing Forms Report - Criteria Selection screen displays.

  3. Configure your report options:
  4. When finished, click the Finish button to run the report.

  5. Click the OK button. Remember to return to the My Reports screen to retrieve your report when it has been generated.

  6. The My Reports screen displays a list of all reports.
