Generate an Audit Forms Report

 

You can generate an Audit Forms report that summarizes the CMS-2728 and CMS-2746 forms submitted at specified facilities within a specific date range.

To generate an Audit Forms Report:

  1. From the CROWNWeb Home screen click Reports. The Facility Reports screen displays.

  2. Click the Audit Forms Report link. The Audit Forms Report - Criteria Selection screen displays.

  1. Configure your report options:

When finished, click the Finish button to run the report. The report runs and the Report Confirmation screen displays.

  1. Click the OK button. Remember to return to the My Reports screen to retrieve your report when it has been generated.

  2. The My Reports screen displays a list of all reports.
