Medication Reconciliation Data Entry in CROWNWeb

The Centers for Medicare & Medicaid Services (CMS) released an updated version of the ESRD Quality Reporting System (EQRS)/CROWNWeb data-collection system. The bulk of this release features enhancements that are designed to support users’ data reporting needs during the 2020 calendar year.

Included in this system rollout was the insertion of the Medication Reconciliation (MedRec) fields on the Manage Clinical screens in CROWNWeb. CMS adopted the MedRec reporting measure for the ESRD Quality Incentive Program measure set, beginning with Payment Year 2022. The MedRec measure assesses whether a facility has appropriately evaluated a patient’s medication.

MedRec data reporting begins January 2020 for all hemodialysis and peritoneal dialysis patients. Facilities are currently not required to report MedRec data via EQRS/CROWNWeb. However, the system currently provides users with the ability to review the MedRec fields, in preparation for data reporting in 2020.
