Medication Reconciliation

CMS recently released an updated version of CROWNWeb, which included the addition of the Medication Reconciliation (MedRec) fields on the Manage Clinical screens.

What is MedRec?

The process of creating the most accurate list of all home medications that the patient is taking, including name, indication, dosage, frequency, and route, by comparing the most recent medication list in the dialysis medical record to one or more external list(s) of medications obtained from a patient or caregiver (including patient-/caregiver-provided “brown bag” information), pharmacotherapy information network (e.g., Surescripts), hospital, or other provider.

CMS adopted the MedRec reporting measure for the ESRD Quality Incentive Program measure set, beginning with Payment Year 2022. MedRec data reporting begins January 2020 for all eligible hemodialysis and peritoneal dialysis patients.

Final PSRs & PSCs for PY 2020 Available

As a reminder, Payment Year (PY) 2020 End-Stage Renal Disease Quality Incentive Program (ESRD QIP) Final Performance Score Reports (PSR) and Performance Score Certificates (PSC) are available to download.

PSRs may be accessed directly from the ESRD Quality Reporting System (EQRS). If you have not downloaded a copy of your PSC, you may request one by contacting the QualityNet Help Desk at: qnetsupport-esrd@hcqis.org or (866) 288-8912. As a reminder, all PSC’s must be posted from January 1, 2020 until December 31, 2020.


ICH CAHPS and Clinical Depression Screening Training

In-Center Hemodialysis Consumer Assessment of Healthcare Providers and Systems (ICH CAHPS)

Each year, CMS provides facilities with the ability to use EQRS/CROWNWeb to attest that they are not eligible for the ICH CAHPS Reporting Measure if they treated fewer than the number of eligible patients over the course of the entire eligibility period.

From January 1, 2020 to March 2, 2020, these facilities are able to use CROWNWeb to attest that they did not work with a CMS-approved vendor to administer the ICH CAHPS survey in 2019 because they treated fewer than 30 eligible patients during the Calendar Year 2018.

 

Clinical Depression Screening

In order to comply with ESRD QIP requirements, facilities must use CROWNWeb to submit Clinical Depression Screening and Follow-Up Plan information for each eligible patient at least once during the calendar year. The Assessment periods are from January 1 to December 31 of each calendar year.
