New Medication Reconciliation Reporting Field in EQRS

March 4, 2024

On January 1, 2024, the Centers for Medicare & Medicaid Services (CMS) added a new medication reconciliation (MedRec) reporting field on the Manage Clinical screen in the End Stage Renal Disease (ESRD) Quality Reporting System (EQRS). Beginning on this date, users can access the new MedRec field by clicking on Manage Clinical from the Patients drop-down tab in EQRS (Figure 1).

Dialysis facilities are required to complete the new MedRec field under the Medication Reconciliation section of the Manage Clinical screen in EQRS for every clinical month, starting with the January 2024 clinical month. Users must select Yes or No from the Was medication reconciliation performed in this clinical month? drop-down field, to complete reporting for the new field (Figure 2). If Yes is selected, the remaining fields in the section are required; if No is selected, the remaining fields are grayed out and will not accept data.

Please note: If your facility batch submits or uses Health Information Exchange/Electronic Data Interchange methods for data submission, please follow the guidance provided by your organization.
