The Centers for Medicare and Medicaid Services (CMS) has announced that the updated Form CMS-2746 is now available to download and preview.
Users can access the newly updated Form CMS-2746 on the CMS.gov website. The updated Form CMS-2746 provides more specific direction when entering data in required fields. CMS also added new fields to the Form to support ongoing Health Equity work.
Please Note: The End Stage Renal Disease Quality Reporting System (EQRS) will continue to generate the existing version 2018 Form CMS-2746 until October 1, 2024. The use of any Form CMS-2746 other than what is available in EQRS is not authorized.
