PY 2025 ESRD QIP Final Reports Available Soon in EQRS

On November 6, 2024, the Centers for Medicare & Medicaid Services (CMS) will release the Payment Year (PY) 2025 End-Stage Renal Disease Quality Incentive Program (ESRD QIP) Final Performance Score Reports (PSRs) and Performance Score Certificates (PSCs). Authorized facility staff will be able to log into the ESRD Quality Reporting System (EQRS) on this date to view and download these documents.

PY 2025 Final PSR

The PY 2025 Final PSR reflects any applicable revisions to your facility’s performance score from those displayed during the preview period. For details regarding the PY 2025 ESRD QIP scoring, please refer to the CY 2023 ESRD PPS Final Rule published on the Federal Register website.

PY 2025 PSC

Facilities are required to download and print both the English and Spanish versions of their PSC and display them in a prominent patient location within the facility. PSCs must be posted by November 27, 2024, which is 15 business days after they are available. PSCs must remain posted throughout calendar year 2025, until next year’s PSCs are available.

Save Your Facility’s PY 2025 Reports

Each facility should save an electronic copy of their Final PSR and PSC for reference, reprinting, and reposting throughout the year, if needed. The Final PSR and PSC can be accessed via the ESRD QIP user interface (UI) in EQRS. For instructions on how to access the ESRD QIP UI, refer to the PY 2025 ESRD QIP UI Quick Start Guide. For additional information about the PSR, refer to the PY 2025 Guide to the ESRD QIP Performance Score Report. Both resources are available for download from the QualityNet website.
